Discover how our home care company process ensures personalised care for you or your loved ones. Although each person’s circumstances are unique, we try to follow the procedures outlined below after making an initial enquiry.
We will make contact to arrange a home visit within 24 hours of you or your family’s request
We will arrange to do a preliminary home visit to assess your personal needs. This will be carried out by one our trained members of the office team who will review your current situation, they will assess what care you may need based on what you are able to do for yourself and then discuss your options with you and your family.
We will work with you to develop a plan of care which will include the number of visits required and outline what care will be delivered at each visit. We will agree times for the visits and give you an idea of who will be visiting you.
We will never start to provide care until you and your family are happy with the plan of care and are aware of how much it will cost. You will be asked to sign your care plan so that we can be sure you are happy to proceed and feel totally in control and involved with your plan of care.
Our aim is to have a small number of carers that will manage your care throughout your journey with us, so that you have a familiar face and build up a good rapport. That way we can be sure that you and the care team are happy and confident with the agreed plan of care.
Our care processes
Your individual notes and care plan are recorded electronically on our App called Pass for care which is safer for everyone . Each day your care team will update your notes and write about any changes, so that everyone can be kept up to date with your progress or any changes to your plan of care. We encourage you and your family to access Pass so that you are always involved and aware of any changes made. We also encourage you to inform your care team about any concerns you may have or any changes you would like to be made to your care plan so they can be updated straight away.
Our care manager and office team will evaluate your plan of care regularly with your care team. This will be formally evaluated after six monthly, or our office team will be checking for any changes in your condition and noting if any adaptions need to be made. This will also be an opportunity for us to check that you are happy with your care team and highlight if you would like us to make any changes on your behalf.
In between these evaluation periods, we are always very happy to provide you or your family with progress updates. This can be undertaken on either a flexible or planned basis.
